Adolescent and Young Adult Oncology Nurse Navigator
Summary
The Waterloo Regional Health Network (WRHN) is seeking an Adolescent and Young Adult Oncology Nurse Navigator (AYA NN) / Oncology Clinic Nurse. This permanent, full-time dual role involves coordinating and supporting adolescents and young adults throughout their oncology diagnosis and treatment journey. The AYA NN focuses on reducing system barriers and ensuring seamless, patient-centered care, providing tailored assessment, education, advocacy, and care coordination. The Oncology Clinic Nurse collaborates in patient care within the clinic, acting as a key point of contact for patients and families, providing education, clinic support, and navigation. Both roles contribute to interdisciplinary collaboration, facilitate access to supportive services, and participate in program development and quality improvement initiatives. The position requires a current certificate of competence from the College of Nurses of Ontario, 3-5 years of current oncology experience, and completion of the Canadian Nurses Association Certification in Oncology (CONC) and the Oncology Foundations Course from the deSouza Institute. Experience with the adolescent and young adult population, care coordination, navigation, and advocacy is preferred.
