Summary

Community Healthcaring Kitchener-Waterloo (The Health Centre) is seeking a part-time permanent Nurse Practitioner for their Outreach program. This role focuses on supporting individuals who are homeless or precariously housed, often with complex mental health and addiction issues. The Nurse Practitioner will provide primary care, including assessment, diagnosis, treatment, and health education, from a Social Determinants of Health and Harm Reduction perspective. The position involves working with mobile primary care teams, participating in HART hub locations, and collaborating with Inner City Health Alliance partners. Responsibilities include triaging clients, maintaining clinical notes, developing health promotion programs, conducting home visits, and responding to emergencies. The role requires flexibility in hours, including evenings and occasional weekends, and necessitates a valid driver's license and access to a personal vehicle for work-related travel.
