Registered Nurse or Registered Practical Nurse Palliative Care Nurse

Summary

The Upper Grand Family Health Team (UGFHT) is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) to join their team as a Palliative Care Nurse. This is a maternity leave contract position with a 0.9 FTE. The successful candidate will provide support to patients and families requiring palliative care, working collaboratively with a Social Worker and other community partners to deliver high-quality end-of-life care. The role involves holistic assessment, symptom and medication management, caregiver support, advance care planning, and interdisciplinary communication. Occasional after-hours support for patients is also required.
