Registered Dietitian
Summary
The Inter-professional Primary Care Team (IPCT) is seeking a Registered Dietitian (RD) to join their permanent full-time team. This role focuses on providing primary care services, with an emphasis on chronic disease management and marginalized populations. The RD will collaborate with the IPCT to plan, implement, and evaluate programs, offering individual and group services, health teaching, and support for chronic diseases like diabetes, COPD, and hypertension. The position involves charting in an Electronic Medical Record and developing health teaching tools and screening programs. Key qualifications include current certification as a Registered Dietitian in Ontario, a Registered member in good standing with the College of Dietitians of Ontario (CDO), and a minimum of 3-5 years of experience in a community health care setting. A Master's degree in nutrition is preferred. Experience in community health centers, outreach, supporting clients with mental health and addiction issues, and certifications in diabetes education or motivational interviewing are considered assets. The role requires knowledge of social determinants of health, adult learning principles, and the ability to develop and evaluate group education sessions. Strong communication, organization, time management, conflict management, and interpersonal skills are essential, as is an understanding of anti-oppression principles and the ability to work with diverse populations. The position requires flexibility in hours, including occasional evening and weekend work, and a valid driver's license with a vehicle for travel within the Brantford-Brant-Norfolk OHT area.
