Summary

Grand River Community Health Centre (GRCHC) is seeking a Primary Care Coordinator to support daily operations and primary care coordination across its locations. This new role will provide direct support to front-line staff, ensuring high-quality care delivery. Key responsibilities include overseeing day-to-day clinic operations, site supervision, staff orientation and training, supporting digital health integration and quality improvement initiatives, and assisting with EMR changes. The coordinator will also integrate AI tools into daily practice. Reporting to the Director of Primary Care, Mental Health and Addictions, this position requires collaboration with the management team and community partners.
