Summary
Brant Community Healthcare System (BCHS) is seeking a Nurse-Led Outreach Team (NLOT) Registered Nurse (RN) for a temporary full-time position. This role involves providing clinical consultation, education, and mentorship to interprofessional teams in Long Term Care Homes (LTCHs) to improve geriatric care outcomes and reduce avoidable hospital visits. The NLOT Nurse will conduct mobile clinical assessments and interventions for residents with acute changes, working collaboratively with various healthcare professionals and community partners. The position focuses on building capacity within LTCHs through education and best practices, while facilitating access to community services. The ideal candidate will have a Baccalaureate Degree in Nursing, be an RN in good standing with the College of Nurses of Ontario, possess a minimum of 3 years of recent clinical experience in acute medicine and geriatrics, and hold current BCLS/CPR certification. A Geriatric Certificate Program (GCP) and training in Gentle Persuasive Approach (GPA) or similar behavioral management approaches are also required, along with advanced clinical skills and a valid Ontario driver's license with a reliable vehicle.
