Social Worker

Summary

The Region of Waterloo is seeking a temporary full-time Social Worker for Sunnyside Home, a long-term care facility recognized for its Butterfly Approach® to care. This role involves applying social work principles to support residents, clients, and their families in achieving enhanced psychosocial well-being. Responsibilities include psychosocial assessment and counseling, developing care plans, providing pre- and post-admission support, assessing admissions applications, and offering professional consultation to multidisciplinary teams. The Social Worker will also intervene in crisis situations, provide information on consent and privacy, facilitate room transfers, and participate in various committees and educational sessions. The position requires a Master's of Social Work degree, relevant experience in long-term care, and knowledge of relevant legislation and healthcare systems. A Police Vulnerable Sector Check and TB test are also required.
